Please bring a valid ID and your current, up-to-date insurance card/s.

NOTE: If you are paying out-of-pocket, payment is due at the time of service via cash, check or credit card.

When to arrive:

Please arrive 30 minutes prior to your first appointment.

This allows our administrative staff enough time to answer any questions you may have and to make sure that all of your paperwork is filled out properly.

Water, coffee and tea are available for your comfort while you wait!

Upon your arrival:

Please sign in and make sure to let the office staff know that you are a new patient!
